Dependent and plans missing during open enrollment

We are running JDE1 9.2.2.2 processing open enrollment.  We have several employees who have plans that are not showing up on the current enrollment statement that are in their DBA instructions.  When the employee gets to the pending changes statement the missing plans are there but other plans are missing.

Also I have an employee who shows a dependent on the current election statement but she is not included in his enrolled dependents for health and wellness.

This is not effecting any particular group of employees - seems to be random.
