Using TFA causes databases to shutdown

We are running Oracle Database 12c Standard Edition Release 12.1.0.2.0 - 64bit Production on Oracle Linux Server release 6.9

The database generated incidents this morning. This document, "Collecting Diagnostics For Oracle Support (Doc ID 411.1)" says to use TFA to ship the diagnostics to Oracle Support.

My problem with doing so is that when a fellow DBA used TFA earlier this year, it shutdown a database that was running on the same server.

Weird thing is that it was a different database on the same server that shutdown.

Has anybody seen this and know of a way to prevent a shutdown?
