Getting error "Changes conflict with another data item. (18,5) " while creating Ad-hoc document

Navigation - FSCM > Supplier contracts > Document management > Add a document page. Select Source transaction - Ad Hoc and create a ad-hoc document. On create document page if we select Create document button.

We get this error and same error also while using import document functionality.

Any help will be appreciated.
