CAFE One - Second Form Not Loading

Hello All,

I created a simple cafe one with two applications...Shared it and assigned to the user...

Issue is whenever that application gets executed...it loads the data only in the first form and not in the second form...Could any one please let me know the exact cause to fix the issue...

Below screen shot depicts the issue ...(Second application highlighted in black color below is not loading )

Tried with standard applications as well & the issue is same...

App release : 9.1

TR: 9.2.2.5
