JDE is showing someone entering today LTD and AD&D for an EE but there is no user id or date updated

how can we see the history on who and how this benefits were enrolled to an employee. the user claims that they did not enroll for an Employee from a benefit group. however when they go to P08334 they see 2 ctgs selected LIFE and AD&D.

please see below how can we identify how this happened. Is this a bug in 9.2. Please can someone help since this is payroll day today and it is a high priority.... thanks so much
