Requisition

I had a situation where a req was created, fully approved, but the req hdr table showed it as pending.  As a result, the requisition could not be found when a PO needed to be created.  This issue happened again yesterday.  So far, I have not been able to recreate it. Has anyone seen this type of issue, if so, what was the cause of it?  We are using PeopleSoft 9.1.
