trying to registry database into dg configuration, this message is returned: ADD DATABASE completed

We have been configuring data guard but when we try to add database this error message is received:
DGMGRL> ADD DATABASE 'ccvdb_mx' AS CONNECT IDENTIFIER IS 'ccvdb_mx' MAINTAINED AS PHYSICAL;
Error: ORA-4088: error during execution of trigger '.'
Failed.
checking drcccvdb1.log file we can see this:
ADD DATABASE
