adop cutover failed with status FLIP_SNAPSHOTS_COMPLETED

Hi,

Need some assistance to resolve the below failed cutover:

Cutover failed on admin tier node with status FLIP_SNAPSHOTS_COMPLETED and failed on web tier nodes with status FS_CUTOVER_COMPLETED.

Log:

" Running cutover phase on node(s): [dbdss07h,oadss16h,oadss18h and oadss19h].
    Output: /psi2/fs_ne/EBSapps/log/adop/70/20181219_154152/cutover/remote_execution_result_level1.xml

Statement handler reference is invalid.

Session killed.
Note that this is normal in the online patching cutover phase.
DB-Handle Error Code: 28
DB-Handle Error Message: ORA-00028: your session has been killed (DBD ERROR: OCIStmtExecute/Describe)
Reconnecting to the database.    [ERROR]     adop phase=cutover failed on Node: "dbdss07h"
