Fluid Candidate Gateway-Online Questionnaire Setup-Random Pre-Qualify Question appearing incorrectly

We are adding a new step for Online Questionnaire for Staff employees as we are implementing Fluid Candidate Gateway with a PUM Update to PUM (18 through 27).   We currently have only the Pre-Qualify Step with 2 screening questions as a step in the Resume.     We created new effective dated Resume Template (added the Online Step), New Job Opening Template, and Screen Template (adding the Online Questions with Screening Quest) all dated 12/20/18 because we want only new Job Openings to show this step and 3 new questions.      When we do the setup for this Online Questionnaire it appears in
