EPM PBCS reporting issues

We are getting errors when running Excel reports in
PBCS.  This just started happening.  We get a series of prompts (see
below).  Attached is a copy of a report after going through the
prompts.  It is missing a lot of data.  Happens on all the reports,
not just this one.  The data is in the system.  The HTML and PDF
reports work just fine.  This is only impacts the Excel reports.
