At time of Kanban Checkout - duplicate Kanban Card gets created?

Hi. My organization uses the Kanban Consumption Screen to perform Kanban Checkouts - we do 2 Phase Kanban Processing (P3157).  I am seeing that once a Kanban Card is Checked Out, another Kanban Card automatically gets created???  I need to find the source of this issue.  I checked Security and I believe the P3157 security setup is ok.  We are on E1 9.20.  Any input would be awesome! Thank you all in advance!
