ZFS Storage Appliance with 18GB or 73GB SAS Logzilla may panic or hang due to an SSD failure

A small number of 18GB and 73GB SSDs used as Logzilla in DS2 or DE2 enclosures have exhibited a failure mode that can affect IO to all other HDDs and SSDs on the same 8-Port 6Gbps SAS-2 HBA port. The SSD failure mode causes the 8-Port 6Gbps SAS-2 HBA to block IO to all other drives on the same SAS port. The failure can affect both ports of the SSD resulting in IO failures on both HBAs connected to these other drives. This can cause panic loops or hangs of both storage controllers.
