ACA 1095-C and 1094-C Questions

Question 1:

Is there a template that is published each year for the 1095-C print form?  For the past 2 years, the existing template's header and footer year was edited using the BI Publisher tool.  If any other change to the actual template occurred, it is not reflected in the template I have access to.

Question 2:

For the 1094-C Build, the R08121A job was ran in proof mode and returned the R08121 1094-C Build Report.  Can someone provide information on what the E - Employee and the F - Full Time Employee mean?

In the R08121A processing options, #4 - Date Option to Consider Employment is set to "B" (Last Day of the Month). There is an employee that was terminated on 11/1/2018 but the R08121 report has an "F" in the Nov and Dec columns.  What date field is used in the application or table to populate the month field on R08121 report?
