Has anyone had an instance where a voucher release contract was put in an open status and all of he

Has anyone had an instance where a voucher release contract was put in an open status and all of he scheduled releases were deleted however a voucher was generated.  If so, can you explain why this can happen and what was done to mitigate it.  Thanks.  We are on image 9.2.25.
