Configuring RADIUS + DUO for DBA's access: 2-factor fails to grant access

Hello,

I am trying to configure RADIUS along with DUO to have a 2-factor authentication process for the DBA team to connect to production databases.

Our database is currently an 11gR2 (11.2.0.4) 2-node RAC running on Intel servers with Oracle Linux 7 as OS.

I have followed this documentation for the configuration: https://docs.oracle.com/cd/E11882_01/network.112/e40393/asoradus.htm#ASOAG040

My sqlnet.ora file for the DB, at the servers, looks like this at the RADIUS configuration section
