Orphaned F4201 records

Started having some issues with orders being entered from P4210 where there are F4201 records and no F4211 records.  In the last 30 days alone, there are 33 instances of this happening. 

I haven't started looking at logs yet, but was just curious to see if there was anything else I should be looking at.
