Target discovery fails to detect DB instance

I'm facing a weird situation when discovering my Oracle 10g instance with OEM 13c.

The target host is hosting several databases from 10g to 11g. Almost all of the database instance got discovered and added successfully to OEM except 1 instance.

I did not encounter any errors during my discovery job & that database home is having the same group as the agent.

Any idea where do I start to troubleshoot on this?
