xml bursting failing with error "ZIP file must have at least one entry"

I have searched and 'googled' until I'm sick of it (about three weeks) and none of what I have found or done has fixed the issue...

RDBMS : 11.2.0.4.0

Oracle Applications : 12.1.3

I see the .zip file on the server with size of 0.  The  actual report is being created fine.  I have stripped the report I am working with down to the 'bare bones' to get it to work AT ALL with no luck.

Here is my control file contents:
