Requisitions being created with same Nonrecoverable Tax value on all lines - some of the time

Hi - we are using R12.1.3 with RPC5 applied.

Before Christmas I worked on an issue about a REQ which had the same Nonrecoverable Tax value for all 19 lines, even though the Amount on all the lines were different.

The user cancelled the REQ and raised it again, and it went through fine the second time. I thought it might be a one off, but then at the end of Jan this year a new call was logged for the same issue with a different requisition. This time the approver noticed all of the lines on a new REQ had the same Nonrecoverable Tax value. The approver rejected the REQ and when it was sent through again, the issue went away.
