Benefits Premiums report

Hello,

every month, I have to send a report to our benefits carrier with the premiums amounts that we are either deducting (such as optional life/spouse/child insurances) or paying for an employee (such as LTD, STD, AD&D)

Is there any report i can run that gives me the benefit plan name and descriptions as well as the premiums?
