HCM 9.2 1095-C forms No mailing address being printed on form.

Has anyone had an issue with mailing address printing on the outside of the 1095 form? Employer address is printing in the ALE box and in the employees name and address box inside Part 1 of the form but shouldn't it also print on the outside in the white space where "First-Class Mail" is pre-printed?

I believe we have all the patches applied but we're missing something.

Any help would be greatly appreciated.
