PO Header Attachments Sometimes Not Printing

I've been searching but have not found an answer to this.  We have PO's converted from requisitions that then have text notes attached to the PO header.  These PO's also go through approvals.  Sometimes, and I have not found the common thread, the notes fail to print on the PO, but most often they do print.  Any help or suggestions appreciated.
