Failure to create 12.1 oracle_home on ODA X7 HA

Has anyone encountered these errors when creating 12.1 oracle_home on ODA X7 HA?

                Message:  DCS-10001:Internal error encountered: Fail to run DB clone.pl:.

The failures start at Clone Db home  task. The ODA has 18.3 databases up and running. 12.1 oracle_home and databases were successfully set up on ODA X7 2M.

Opened an SR, but still no resolution.
