Duplicate BIL rows

We have a scenario where an SFA analysis type row created   2 BIL analysis type rows with the same Trans from id. The SFA analysis type was created from an ACT analysis type. This is strange and a scary scenario. Did anyone face this and know anything about root cause and/or to prevent in future from happening? We don't have any customization for GM_GMFACS and the pricing process. This has not occurred in the past in our 9.2 PS we are at PUM 29.
