Media attachments lost in Model WO

in 9.2, it seems that our Model WOs have somehow lost their attachments, causing the PM WOs to be created without attachments.

I have gotten the date range down to about a 30 range...Same model created a PM WO on 1/14/19 with attachments and a PM WO with 2/21/19 without attachments.  Model of course now does not have attachments.

Any ideas on how to trouble shoot or have you seen this before?
