Database Install/Upgrade/Opatch (MOSC)

ODA X3-2 version upgrade 12.1.2.9.0 to 12.1.2.12 fail with GI upgrade.

Hi  While I was upgrading ODA (patch no 26433712)  step: 4

/opt/oracle/oak/bin/oakcli update -patch 12.1.2.12.0 --server -local

failed with GI patching.

I tried multiple times after

/opt/oracle/oak/bin/oakcli update -patch 12.1.2.12.0 --clean

/opt/oracle/oak/bin/oakcli update -patch 12.1.2.12.0 --server -local

But no luck. Any one know why is this .

All other components Controllers, ilom, bios , ipms ,oak, ipmi,oak . But all updated  except GRID

GI log says

2019-03-23 11:41:46: Successfully started the agent from Home : /u01/app/agent/core/12.1.0.5.0

2019-03-23 11:41:46: SUCCESS:  Successfully started the EM Agents

2019-03-23 11:41:46: ERROR:  Unable to apply the GRID patch
