Planning security issue with returning user

Strange issue with a returning employee in Hyperion Planning.  We recently had an employee return to our company that was terminated less than 1 year ago.  We added the user back into one HSS group that is provisioned for Hyperion Planning application.  However, when the user tries to run a report they get this error:  "Cannot open cube view.  Essbase Error (1054060).  Essbase failed ot select application [APPNAME], because [user.name@MSAD] is not completely provisioned by Planning."

We tried all the standard items including removing user from group, removing the security filters in planning, manually removing the security filter from EAS console, refreshing the database over and over, removing unprovisioned users from Planning, etc.  We even ran a Maxl to remove the user from Essbase (drop user user_id from security_file;  (or) drop user User_id;).
