We are not able view some organization in org_organization_definitions table and those organization

We are not able view some organization in org_organization_definitions table and those organization data reflects only when apps initialization done. Can any one tell what cloud be the possible reason, How we can fix it permanently as it is impacting some of the custom webadi which are failing due to unavailability of organization in org_organization_definitions table.
