Benefit Details Summary Page for 401K

After applying PUM 29 when the 401K is selected from the Benefit Details page to view the summary page, the Customer Service and Extension text fields are visible.  In PUM 25 there is a bug/fix, 2231292.1, stating the fields are now hidden.  But we are seeing the fields.  Has anyone else noticed this?
