PO approval shows error with "Wait for Concurrent Program" activity and "#NORESULT" activity result

PO approval workflow showed error and says "Wait for Concurrent Program". The PO was supposedly be approved by the top approval but did not show on the notifications and instead gave this error message.

What action should be taken?
