Login Trigger to Capture Username with Terminal IP

Hello,

In our database we have multiple databases but no confirm from which terminal they are being used, although I have checked terminal IPs in listener log file but they are just IP I can't see which user is being login from which IP, so I want to create a login trigger which should capture username with its client terminal IP, please support.
