Dear Experts

in our production environment with Field service request forms every single click is resulting in ORA-01403, the recent changes made to system were forms bundle patch 10.1.2.2 and database upgrade to 11.2.0.4.

This is the only form being degraded and affecting many seeded business functionalities

any help would be much appreciated please
