iscsi session(8) unable to enumerate logical unit - inquiry failed

I have an LDOM with non-global zones running on it all within a T5-8 SuperCluster.  This is a production and live environment supporting real customers.

We have external ZS4 storage arrays connected to this SuperCluster. Recently, we have to perform a "takeover" task on the ZS4 storage appliance due to the active head experiencing some issues. After the failed memory riser card in the ZS4 was replaced, we did a failback to the original active head. Everything seems to be well except for one thing. One of the LDOMs hangs on commands such as "echo | format" or "zpool import" inquiry.
