Attachment Icon not Turning on when Attachments are enabled

Hello,  I am having an issue with the attachments function.  I had a custom form that had attachments enabled.  When some modifications were done to this form all of a sudden the attachment icon won't turn on.  I have checked the attachment setup and nothing has changed there.  I have another custom form that I just created the attachment function on and again the icon is not turned on.  I am on EBS 12.2.4.  Does anyone know what changed?
