Two Issues - Unable to active any BE and autoboots to maintenance mode

We have 3 BEs, one of which we cannot remember passwords since it's so old.  We recently created a new BE (applied CPU from January) but after luactivate and init 6 the server hung up at trying to boot an even older BE that the system no longer has but is somehow in the menu.lst file, and the new BE was not in the menu.lst.  So, we attempted to boot to a failsafe mode to attempt the instructions to return to a previous BE, but the commands did not work. Plus failsafe mode kept alternate mounting the 3rd oldest BE
