Can anyone point me to the latest 1095-C/ 1094-C correction and replacement document please?

HCM 9.2 PUM 23 - we have a couple of errors in our employee ACA data we are trying to correct. We need to resend the corrected files to the IRS but we cant find a current doc on procedures for correcting and re sending the files.
