Custom Print with Dynamic Image from Shell Attribute Form

We have a Custom Print that pulls in the Image from the Shell Attribute Form.  However when that Image is changed, and the Custom Print is re-run/reprinted, the Image doesn't change.  Any ideas?  Thank you!
