Oracle RAC Migration across server and keep hostname/IP same

Hi,

We have 3 node RAC environment(EBS Database) on RHEL 6.1. GRID version is 12c and DB version is 11.2.0.4. There is no ASM in source/target.
We need to migrate DB to a new hardware(RHEL will be upgraded to 7.6) and Upgrade DB to 12c. We need to keep the hostname and IP address same as source server so that there are not much configuration changes.
What approach can we follow for this? What configuration changes are required for GRID infrastructure and when to implement this.
Till now we came up with this approach:

1. Install GRID 12c and configure CRS at target server
