Life Event Upload Document Error When Denying

When an employee uploads 2 documents for a Life Event, the Benefits Admin approves one and attempts to deny the other we get the Security Exception error message below.  This is happening more frequently.  Has anyone encountered this error message? Do you know what causes it? If so, what did you do to resolve it and prevent it in the future?
