Cafe One - Need to display Avalara tax rate on Address Book

Hi,

I am trying to bring up the tax rates based on the zip code on the Address Book. I tried the following two links, by mapping the zip  code and state description. But both bring a blank screen on the Address Book application.

Result is the following... Blank in the Cafe One frame.

URL Verification is successful...

Get the following when I click on Pop-out content, which tells me that it does go and pull up the correct link. It just doesn't display it on the Address Book form (previous screen print).
