How to query the W-2 Consent option to see who's opted out of paper?

Has anyone written a query that will export the employee's name and the selected W-2 consent option selected (online, paper, or both)?  If so, can you share that query?  We have not implemented the electronic consent function yet but staff has been asking about it.  If we implement, I'd want a way to get a list of staff and what they selected.  I didn't see the result stored in the People screen anywhere.
