Item getting counted more number of times during cycle counting

In my current organization , the cycle counting for A items is set to 12 times an year , For B items is set to 4 times an year , for C items is set to 2 times an year.

But for some unknow reasons the cycle counting is happening more times for example Item which belong to class C is getting counted for 30 times.

Can some one guide me what could be the reason.
