No Prospect Record Created by Application Transaction Mgmt

We have a scenario where two applications being moved from Application Transaction Mgmt > Application Transactions to Recruiting/Admissions had applicant records (adm_appl_data) created but no prospect records (adm_prspct_car).  I am told this is not typical behavior.  What might cause this and how might I go about troubleshooting?
