How to Add Note Field (DFF) to GL Segment Value Changes

We need to add another field/DFF or preferably another pop-up as shown below for a note field to explain when an account segment was disabled or enabled.

Please advise on how to best accomplish this and if you have other suggestions. Much appreciated.
