Sales order line is fall off delivery and closed with zero shipped qty

Few Sales order lines in a day are falling off delivery and getting closed with zero shipped qty. This happens while ship confirm or during picking. We realize this only after ship confirm. This happens when there are multiple sales order lines are one delivery and one line gets dropped of delivery

What could be the possible reason and how to reproduce this scenario?
