Dependent LOV shows correct values but doesn't copy selection into the Excel cell

We are using 12.2.5, and have set up a dependent LOV.   The LOV itself shows the correct values, which is great.  However, when we select a value from the list, the LOV closes but the selected value isn't put into the Excel cell.  It is the same whether we do regular select or quick select. 

I really appreciate any suggestions.  Dependent lists will be very useful, and we really want to get it to work.
