ePerformance evaluation with enabled combo boxes

Hi team,

I am looking to create a section that I can pull into a document template  that the user can select one to many values and have it be able to display on the delivered printed evaluations.  For example:  give the manager a method to select one of more of the following:

- Promotion

- Merit Increase

- Status Change

- Other

So far, I did the following:

1.  Create a content type with multiple yes/no fields. 

2.  Created a content item associated with that content type. 

3.  Created a new section in ePerformance

4.  Added the section to an ePerformance template and enabled the item.
