Does an increasing amount of items have a negative affect in performance?

Hi everyone
I am Wondering if anyone can assist me.

We have an ever increasing number of items in out system (currently 1.6 mil rows however only 390,000 are unique items and split across multiple orgs) and I have a few questions.

1. Does having a huge number of items in the system have any impact on performance generally (for instance we have users complaining about time taken to add items in the Quote form).

2. Would de activating the items have a positive affect on performance

I have searched though forum but cannot find any information which would help me.
