9.2 TAM - Submitted Resume with no answers

We configured our CG --> SITE to make answers required for all questionnaire.

Issue - There are times when an applicant has successfully submitted resume, however, none of the answers that were provided by the applicant were saved to the system. While we were able to replicate this issue (continuing application process, ignoring "Data Integrity Error"), we are unable to understand why this happens at all.

Has anyone encountered similar issue? Is there a solution or workaround, something we were unable to find on Oracle Support site.
