User connections while applying patches

We have different types of user connections, from application servers, third-party tools, direct connections.  When applying a patch, what is your normal best practice to handle connections?  Changing user passwords?  That's a lot.  Stopping application servers?  That would get some.  Or, is it really nothing that you worry about as long as it's advertised that the database will be in maintenance?
